New Patients

Expect the first visit and exam to last from 1 ½ to 2 hours. All new patients will receive a complete history, review of systems and examination. A full history and exam will be done for all new patients, regardless if they just want to get in for a quick adjustment.

If the doctor feels it is appropriate, you might have treatment on the first visit, to include chiropractic manipulation, rehabilitation. Your diagnosis will be explained to you as well as how it relates to your symptoms. If you have any questions, please ask.

If needed, diagnostic imaging or blood tests will be performed at a local facility.

Please be sure to bring your driver’s license, any primary or secondary insurance information, your payment and a calendar/appointment book with you. If accurate insurance information is given over the phone, the fee charged will be more accurate for the first visit.

Below, please download and print out the new patient forms, the informed consent forms and any forms that are associated with your current problem listed. Filling them out before your first visit will save time at the office.

If you are interested in seeking treatment for Female Hormone imbalance issues, then please print and fill-out the Female Health History Questionnaire below. Women making appointments for neuromusculoskeletal problems like low back and neck pain do not need to fill out this form.

Harmony Chiropractic Center, Inc. accepts several types of insurance. For those who do not have chiropractic coverage on their insurance or those that wish to see Dr. Royer as an out-of-network doctor, the services are reasonably priced.
